Toxoplasmosis-Old scar with new reactivation

This 32 year old Hispanic male presented with a black spot in his vision, right eye. He gave a history of having an infection in his right eye several years ago in South America, and was treated with antibiotics. Upon examination, an old Toxoplasmosis scar was found in the periphery of the right eye (yellow arrow), along with an active area of retinitis (blue arrow). He was put on double strength sulfamthoxazole/ trimethoprim twice daily for two months, which led to regression of the active area of retinitis.
