Endogenous Endophthalmitis - Drug Abuse

A 30 year old female complained of painless blurred vision of the right eye. The visual acuity was counting fingers. Fundus examination showed the findings captured in this montage photograph. She had macular subretinal fluid and a subfoveal white infiltrate with scattered intraretinal hemorrhages. Further questioning revealed that she injected drugs intravenously. She was treated with inpatient intravenous antibiotics without vitrectomy and recovered vision to 20/200 in the eye. She was left with a submacular chorioretinal scar.
