Hydroxychloroquine Retinopathy Screening - Unreliable 10-2 Visual Field Test
Unreliable 10-2 visual field(VF). This is the 10-2 VF from the right eye of a 79 year old woman taking 200 mg/d of hydroxychloroquine for unspecified arthritis. She was 63 inches tall and weighed 125 pounds, therefore her dosing was nontoxic. There are many flags that this test is unreliable. The fixation losses were 5 of 18 trials (blue arrow). The false negative responses were 15% (green arrow). The false positive responses were 15% (orange arrow). Therefore the various indicators of abnormality, such as the depressed mean defect (brown arrow), pattern standard deviation (red arrow), and paracentral scotomata (green-circled and red-circled locations), must all be interpreted with skepticism.
